过渡关心 is the coordination of your healthcare once you leave the hospital. 我们项目的目标是提供完整的护理，以防止您再次住院.   

You are eligible to participate in this program at no cost if…

• you are a Kershaw县 resident
• you have no health insurance AND
• you are hospitalized or placed under outpatient observation at 音乐 Kershaw Medical Center.

What can I expect from the 过渡关心 Team?

• 一个私人教练，帮助协调你出院后的医疗护理
• Prevention of unnecessary hospital readmission through patient education 
• Help with finding a Primary Care Provider
• Assistance in managing medications
• Possible enrollment in other assistance programs
• Regular contact with your assigned Community Care Coordinator/Case Manager
